I built a self-hosted EMR for a single clinic, including reception, vitals, consultation, observation, lab, and pharmacy, as seven role-gated portals around a live patient queue, running on a single clinic machine and reachable from staff phones over the LAN.

The core decision was to make the visit's status the queue itself, not a separate table, so the two can never drift; every status change writes an event that reconstructs the encounter and doubles as the audit trail. Color is reserved for clinical meaning: amber only for readings a clinician flags, red only for documented allergies, because tinting everything teaches staff to ignore it.

For consultation notes, speech is transcribed on the clinic machine, so audio never leaves the building; only the transcript goes to a cloud LLM API, which drafts a summary that the doctor edits and signs off on before it's saved.

I also designed the permission model (26 permissions composed into configurable roles), Docker deployment with HTTPS over the LAN, and tests for queue transitions and concurrent token allocation, which caught a race condition in my first implementation.
