Members with chronic conditions often rely on emergency rooms and inpatient care because of unmanaged symptoms and limited early intervention. This reactive pattern increases avoidable costs and hinders better health outcomes. The plan must focus on proactive identification and early support to stabilize conditions and reduce preventable acute care utilization.
I developed a case management platform by leading a multidisciplinary team, including clinical health guides, a medical director, and non-clinical guides, to deliver proactive outreach, navigation, and benefit support. I referred members to social workers for financial and community resources and strengthened care coordination by improving follow-up compliance and directing members to the appropriate level of care. I also provided broad health education to enhance literacy and adherence, ensuring medical director oversight through case rounds and ongoing consultations with case managers.
I managed 35,000 complex risk members using a disease- and vendor-agnostic model, achieving a 12 – 20% claim trend reduction ($2,400 – $5,000 savings per member), 45% reach, over 92% engagement, and an NPS of 80.

Large employers offer multiple health plans, and members often switch annually. This leads to siloed operations, fragmented care, and gaps in preventive and chronic care management, increasing the risk of high-cost ER and inpatient use and limiting a consistent member experience.
I developed a multi-carrier advocacy platform that unifies clinical and non-clinical member services across digital and contact center channels. By leveraging analytics to identify preventive care gaps, the platform guides members through engagement. At the same time, automated workflows and scripting enable scalable, condition-based care and wellness interventions—delivering a seamless, proactive, and high-value member experience.
I managed a million members, closing 400,000 preventive and chronic care gaps to improve adherence and continuity, achieving high engagement and an NPS score that reflects strong member satisfaction and experience.

To enable AI-driven transformation, reduce cost, and improve agility, the organization must modernize and migrate to cloud-based architectures.
I developed initial R-dispositions and an end-to-end modernization strategy to prepare the application portfolio for cloud readiness. I also created a comprehensive cloud pro forma outlining current infrastructure costs, modernization investments, and steady-state cloud run costs. I crafted an executable migration wave plan that incorporates tokenization, security, and vulnerability remediation, as well as operational dependencies, to ensure a secure, scalable, and sequenced cloud transition.
I migrated 238 applications to the cloud, rehosted non-modernized systems for continuity, and reduced overall operating costs through cloud adoption and infrastructure consolidation.
